Second Valid US Passport

To obtain a Second Valid US Passport quickly, applicant must meet the following qualifications:
.

-- Applicant has US passport in his or her possession, AND;

-- Applicant's passport was issued within the past 15 years AND;

-- Applicant was 16 years or older when passport was issued.

-- Applicant's passport is not damaged.


If applicant does not meet the above criteria, please apply for a New US Passport. .

Applicant must gather documents listed below including original name change documents to send to us for processing.

1. Your Passport
    Send Sierra PVS your current 10 year Passport. We will return your current 10 year Passport along with your new Second Valid Passport, which will be valid for 2 years. The new 2 year Passport will have a different Passport number.

2. Passport Photos
   
You will be required to submit two (2) identical passport photographs. Photographs must be taken by a passport photographer. Home made photos are not acceptable and most often cause a delay in passport issuance.
 
   
3. DS-82 Application (online Application)
   
Click on the link at the bottom of this section (Section D) to create a DS-82 Application for Passport by Mail.

Print all pages of the DS-82 form, including the instructions, so that each page is printed only on one side of the paper. Do not print on both sides of any sheet of paper.

You must sign the completed DS-82 on the last page of the DS-82 form.

4. Government Fees
   
A check or money order made payable to "U.S. Department of State" for $170. This fee may also be paid online via credit card when filling out our online order form below.

NOTE: If paying by check:

Please include your date of birth on the memo line of the check. Starter checks for new bank accounts are NOT acceptable.

Although it is recommended that your check have your name pre-printed on it, if submitting a business check or someone else's check, make sure to include your name, address and date of birth on the check. Money orders must contain your information on them.


 
    5. Letter of Authorization & Declaration
   
A letter of Authorization gives us the authority to submit your application and pick-up your passport when issued.


NOTE: PLEASE SUBMIT 2 COPIES. Each copy MUST contain your original signature. Fax or photo copies of your signature are NOT acceptable - both letters must contain original signatures.

6. Proof of Departure
   
Submit a copy of your computer-generated itinerary or your ticket from an airline, travel agency, or online booking agency showing your departure from the U.S. FAX or photocopies are acceptable. If you are unable to provide a trip itinerary, we can still assist you without one. It may take one or two additional days to process your request.
 
   
7. Passport Photos
   
You will be required to submit two (2) identical passport photographs. Photographs must be taken by a passport photographer. Homemade photos are not acceptable and most often cause a delay in passport issuance.

1. Order Form
  You must complete the Order Form. Please print two copies of the confirmation page. Include one copy in the envelope you ship to us and keep one copy for your records.

Processing times are business days and do not include shipping time. Keep this in mind when placing your order. Orders are filled based on departure date. For example, a traveler leaving in 10 calendar days that selects 5 - 7 day service may experience fulfillment times of up to 11 calendar days including shipping.

ups The UPS Shipping Document must indicate "UPS Next Day Air". Any other type of UPS service is NOT acceptable.

USPS For shipments via USPS Express Mail or other shipping providers, please follow the instructions carefully when completing the air bill.
