You would need to submit a DS-64 and DS- 11 which are application forms for a lost or stolen passport. You can download these applications from www.allstarpassports.com. After you have filled out all of the necessary application forms and provided us with the proper documentation (itinerary, passport photos and authorization letter) you would mail them to us ( with your choice of mailing service- Fed-Ex, UPS, DHL or USPS) and we would take them into the passport office and expedite them according to the service you requested.
