| Photo must meet specs and be 2x2, color or black and white, with a standard white or light-colored background. Please do NOT send digital photos as they may not be accepted and could cause delays. AAA, Kinko's, Mailboxes Etc and most large chain operated pharmacies (such as Walgreen's) provide passport photos |

| Include a travel itinerary and/or copy of your airline tickets. Travel must commence within 90 days of your visa application. |
