Parents Must provide notarized proof and consent:
| If the parent(s) is traveling, then they must sign and notarize the application of the minor, submit a notarized copy of the minor’s birth certificate and be applying for a Brazil Visa simultaneously, along with the child.
If the parent(s) is NOT traveling with the child, but already has a Brazilian visa then they must provide a photo copy of that visa as proff and send it along with the application
If the you, the parent, have sole custody of the child, you must show legal documentation of such custody. |

Valid Passport
| Pack and ship your valid passport with at least 6 months of validity left and at least one visa page. |

1 Passport Photo
| Photo must meet specs and be 2x2, color or black and white, with a standard white or light-colored background. Please do NOT send digital photos as they may not be accepted and could cause delays. AAA, Kinko’s, Mailboxes Etc and most large chain operated pharmacies (such as Walgreen’s) provide passport photos. |

Travel itinerary
| Include a travel itinerary and/or copy of your airline tickets. Travel must commence within 90 days of your visa application. |
